- To provide persons and their families with an easy and effective way to obtain, coordinate and monitor care services, regardless of where they or their families reside.
- To maximize the functional independence of individuals with chronic or short term disabilities. Quality care management will be designed to help individuals and their families become as independent as possible.
- To maximize quality life of disabled individuals by providing quality care in the most appropriate and least restrictive environment possible.
- To maintain the family as a key support system whenever possible. The family has in the past and will continue to be the single largest provider of care services. Effective care management enhances family support rather than replacing it.
- When family is not available, or when desired and needed by the client/client’s family, to replace the family as a key support system.
- To ensure efficient use of resources. Care management will help achieve individual and family goals by the most cost effective means. This is done through identifying, coordinating, and managing an often complex and confusing array of services and resources.
